Please note:
- You only need to fill out Section 1 in the I-9.
- Please make sure you sign the W4.
- Please email the substitute reference form to three of your references and have them email it back to Cathy O’Dell at COdell@spartanburg3.org. Do not fill out the reference form for yourself!
- The DHEC TB form must be filled out by a physician.

If you’d prefer, we have paper copies available for all forms at the District Office (3535 Clifton Glendale Rd.).
